What is neural fascial prolotherapy?

Neural fascial prolotherapy, also known as perineural injection therapy or subcutaneous prolotherapy, is an innovative pain treatment focusing on nerves located just below your skin at your pain site. These nerves are responsible for sending pain signals to your brain, causing the discomfort you feel each day.

When these nerves get inflamed, they can do more than just cause pain in the immediate area—they can cause degeneration of the surrounding area, such as your spine, knee, hip, ankle, or shoulder. This can lead to future health complications that can cause more pain and require further intervention.

Dr. Truong injects a small amount of sterile water and dextrose (a form of sugar) into the affected nerves. Because the needles are very small, your treatment will be virtually painless.

These injections stimulate growth factors which naturally heal and restore nerve health. When nerve function is normalized, the pain dissipates, often permanently, after a number of injections. Many patients report pain relief immediately.

How do we use neural fascial prolotherapy?

Getting chronic pain treatment is usually fast, easy, and convenient. First, you’ll meet with Dr. Truong and discuss your medical history, your current condition, and any medications you’re taking. This step is crucially important to ensure your treatment’s safety and effectiveness.

Some patients may have medical situations that can make them less than excellent prolotherapy candidates. These include:

On the day of your treatment, you’ll lie down in a comfortable position and receive a series of small, nearly painless injections at your pain site. This procedure requires no anesthesia, and you should feel only minor discomfort. If you feel any discomfort, you will be given a numbing spray to eliminate the injection pain. This spray does not affect your treatment effectiveness in any way, and it does not increase your risk of side effects.

The injections contain a mixture of sterile water and medical-grade dextrose or mannitol. Both are sugars derived from natural plant sources. The injections work because when your muscle or nerve gets inflamed, it releases pro-inflammatory substances that activate the transient receptor potential V1 (TrpV1) which causes nerves to release inflammation-causing substances. This is where your nerve pain originates.

Dextrose and mannitol bind to and inhibit the TrpV1 receptors, eliminating your pain and helping to restore your nerves to normal, painless function.

Most patients report at least some pain reduction right after their treatment. In many cases, patients respond to 5-8 treatments, depending on the severity and duration of their condition. You can expect treatments once every 1-2 weeks to achieve long-term pain relief.

As with any medical procedure, results of these treatments will vary from patient to patient, depending on age, genetics, severity of your condition, and other health factors.

What are the potential side effects of neural fascial prolotherapy?

Most patients report mild or no side effects after treatment. However, you may experience minor side effects following your treatment. If so, they should resolve within a day or two. If they don’t improve, or worsen, contact Dr. Truong right away.

Common side effects include slight pain, redness, bruising, or bleeding at injection sites.

Severe side effects are very rare. But if you experience any of these side effects, don’t wait—get emergency medical attention right away:

  • Dizziness

  • Extreme pain at or near the treatment area

  • Fainting

  • Fever

  • Infection or sores at the injection site

  • Nausea and vomiting
